What to Bring to a DOT Physical Exam


• your valid driver’s license

• your previous CDL card

• a list of your medications, prescription and over-the-counter, all of them, including dosage

• eyeglasses, contact lenses,or hearing aids, if you wear them

• If you have diabetes: results of your most recent Hemoglobin A1C (HgA1c) and your blood sugar logs

• If you have sleep apnea treated with CPAP: bring readings from your machine documenting 90 days (one year is better) of wearing it for more than 4 hours per night for more than 70% of nights

• If you have heart-related issues: a letter from your cardiologist that describes your heart condition, related medications and indicates you are safe to drive a DOT vehicle; also bring results of any recent stress tests, echocardiogram, or other heart testing completed within the past 1-2 years

• If you have suffered a stroke, a brain tumor, seizure disorder, or bleeding in the brain: a letter from your neurologist that describes your neurological history, related medications, and current neurologic and psychiatric state and stability and indicates that you are safe to drive a DOT vehicle

• If you have experienced the permanent loss of use in an arm or a leg: a letter from your physician describing the injury and if you have any work restrictions due to the injury; also bring a copy of any Skilled Performance Examination that has already been completed

• If you are taking any medications that may cause sedation or sleepiness or controlled substances (includes narcotics, sleeping pills, anxiety medication, ADHD medication):  a note from the doctor prescribing those medications regarding the safety of driving a DOT vehicle while using these medications

• If you are taking the blood thinner Coumadin (warfarin): the results of a recent INR blood test and letter from your doctor indicating the stability of your INR level on treatment

These are guidelines intended to help make your visit easier. Additional documentation may be needed as determined by your examiner.

Please be prepared to give a urine sample, take a vision and hearing test and have a complete examination.
